a first suture attachment configured for fixed attachment of the first suture to the shell;
a shuttle suture having two ends, one of the two ends having a suture loop configured to receive the first suture and the other of the two ends having a free end;
an internal locking mechanism in the second passage of the shell configured to receive and permit at least one of the first suture and the shuttle suture to pass therethrough in only a predetermined direction and configured to tighten against at least one of the first suture and the shuttle suture moving in a direction opposite to the predetermined direction;
the internal locking mechanism including a woven suture; and
the shuttle suture passing in a first direction in the first passage and looping in a second direction into and through the internal locking mechanism, wherein the first direction is generally opposite of the second direction;
